Scope of Services
DMS Services Staff:
Nurse Practitioner: Donna S. Westervelt, CRNP, MS, CDE
- Responds to requested consults from physicians with
known or newly diagnosed diabetes mellitus
- Initiates a glucose management plan for the duration of the patients' admission
- Sees follow-up consults to make adjustments in the initial glucose management plan
- Documents diabetes management plan in the inpatient record on a daily basis
- Sees patients in outpatient clinic at least two mornings a week
- Implements and oversees inpatient diabetes management protocol development
- Works collaboratively with inpatient endocrine attending, fellow, case managers,
diabetes educators, pharmacists and visiting home health care nurses
- Assists in the coordination of research initiatives that may result from the
management service, including applications for approval by the Institutional
Review Board
Dietitian/Certified Diabetes Educator: Susan Yesavage, MA, RD, LDN, CDE
- Conducts thorough assessment to determine knowledge deficits and improper
self care behaviors
- Provides education appropriate to patient's needs and stage of change with
focus on survival skills
- Communicates recommendations to appropriate staff
- Serves as a resource to staff and patients
- Provides link to outpatient services
Clinical Data Analyst: Angela M. Green, B.A. Health Science & Policy
- Manages, organizes and reconciles data entered into the DMS database
- Creates monthly statistical reports
- Analyzes outcome measures such as length of stay, readmission rates and
resource utilization
